The LCDC II will provide direct services for consumers, and complete associated documentation for the consumer and program.
Ensure clinical documentation is submitted in accordance with Center standards and meets all designated requirements
Link and refer consumers presenting to clinic to appropriate services (internal/external)
Comply with and participate in Quality Assurance and Quality Improvement processes, and with required contractual and Center policies and procedures
Other duties as assigned
Proficiency with standard Microsoft Office applications and system databases
Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
